Why it matters

Preoperative vulnerability to delirium, cognitive decline, and reduced survival after major cancer surgery is not random but reflects coordinated dysregulation across circadian, gut-brain, autonomic, and inflammatory systems. Identifying these phenotypes could enable targeted prehabilitation and risk stratification.
