Why it matters

Intermediate-high-risk PE carries a substantial risk of early hemodynamic deterioration despite anticoagulation. This trial provides high-quality evidence that catheter-directed thrombolysis can markedly reduce that risk without a significant increase in major bleeding, offering a potentially safer alternative to systemic thrombolysis for selected patients.
