Why it mattersFrailty is a major perioperative risk, and prehabilitation is widely promoted to improve outcomes, but evidence on functional independence is limited. This large trial shows that a short, supervised multimodal program did not reduce care dependency at 12 months, challenging assumptions about universal prehabilitation for frail older surgical patients.
