Why it matters

The widely held assumption that physical activity protects brain structure and cognition is challenged by these null findings from a large, well-characterized cohort. Clinicians in neurology and performance longevity should be aware that self-reported activity may be unreliable in older adults, and that observed brain changes may drive activity levels rather than the reverse.
