Why it matters

Selecting patients for bridging thrombolysis before thrombectomy remains challenging. This analysis suggests that patients with larger baseline ischemic cores (ASPECTS <8) may derive functional benefit from tenecteplase, whereas those with smaller cores may not, and could face higher mortality. These findings could inform patient selection and highlight the need for prospective validation.
