Intra-Arterial Alteplase After Successful Endovascular Reperfusion in Acute Stroke: The PEARL Randomized Clinical Trial.

JAMA·October 13, 2025·PMID 42262770

Why it matters

Despite successful mechanical thrombectomy, many stroke patients have suboptimal recovery. This trial provides evidence that adjunctive intra-arterial alteplase may improve excellent functional outcomes, addressing a persistent gap in post-reperfusion care.
