Anemia is common after aneurysmal subarachnoid hemorrhage and may exacerbate brain injury, but optimal transfusion thresholds remain uncertain. This large, multicenter trial provides high-quality evidence that a liberal strategy does not improve neurologic outcomes, supporting current restrictive practices and reducing unnecessary transfusions.
