Individualized Cardiac Index-Guided Hemodynamic Therapy in High-Risk Patients Undergoing Elective Major Abdominal Surgery: The PELICAN Randomized Clinical Trial.

JAMA·October 19, 2026·PMID 42896534

Why it matters

Intraoperative hemodynamic optimization is common but often guided by generic targets. This trial shows that personalizing the cardiac index goal to each patient's preoperative resting value does not improve early postoperative outcomes, suggesting that such individualized targeting may not be necessary for high-risk abdominal surgery.
