Targeted left ventricular lead placement in biventricular pacing for heart failure: a national, multicentre, double-blind, randomised controlled trial in Denmark.

Lancet (London, England)·August 31, 2026·PMID 42673980

Why it matters

Left ventricular lead position is considered a key determinant of CRT response, and observational data suggested targeting late electrical activation improves outcomes. This adequately powered trial provides definitive evidence that such targeting does not reduce death or heart failure hospitalization compared with standard posterolateral placement, and may increase lead-related complications.
