Acetic acid chromoendoscopy for detection of neoplastic Barrett's oesophagus: stepped wedge cluster randomised controlled trial (ACID-trial).

BMJ (Clinical research ed.)·October 21, 2026·PMID 42907095

Why it matters

Barrett's oesophagus surveillance aims to detect dysplasia and early adenocarcinoma, but standard random biopsies may miss subtle lesions. Acetic acid chromoendoscopy has been proposed to enhance mucosal visualization, yet this large pragmatic trial found no benefit in community practice, suggesting that adding AAC to routine surveillance may not improve neoplasia detection.
