Why it mattersCurrent guidelines often reserve DMT escalation for multiple silent lesions, but this study shows that even a single silent lesion portends worse outcomes and that escalation reduces relapse risk. Neurologists may need to reconsider the threshold for switching therapy in clinically stable patients with new MRI activity.
