Why it matters

For high-ischemic-risk patients beyond 12 months after stenting, the trade-off between ischemic and bleeding risk with antiplatelet strategy is critical. This trial quantifies that trade-off, showing clopidogrel monotherapy reduces bleeding but increases ischemic events compared with extended DAPT, with no net clinical benefit.
